The hospital discharge reinvented

With our Vaistat Hospital module, each discharge is treated as a crucial step in the treatment journey. As soon as a patient is discharged, the digital prescription is automatically transmitted to the designated pharmacy, which can then activate a delivery or support request. This step can be handled either by a Health Proximity Agent (APS) or by the pharmacy’s assigned medical courier. Both profiles are integrated within the same ecosystem, ensuring smooth, compliant, and secure coordination between the different stakeholders in the journey. Thanks to this integrated approach, the patient receives their medication, instructions, and reminders before even arriving home, while ensuring complete traceability of each step—from leaving the hospital to confirmation of receipt at home.

The Post-Discharge Support Program (PAP)

At the core of the system is the Post-Discharge Support Program (PAP), an exclusive innovation by Vaistat. This module ensures proactive and personalized follow-up between hospital discharge and the first medication intake.
The PAP features include:

  • Automatic transmission of prescriptions via e-Rx API between hospitals, clinics, and pharmacies.
  • Management and coordination of hospital discharges.
  • Automatic assignment of a PHS (Proximity Health Agent) or medical courier.
  • Real-time tracking of deliveries, validations, and receipt confirmations.
  • PHS mobile application with secure scanning, patient signature, and full traceability.
  • Patient application: notifications, medication reminders, renewals, and NFC/in-app payments.
  • Export of dosage and therapeutic instructions directly to the patient profile.
  • Integration with pharmacy RX systems and automatic updating of courier accounts.
  • Integrated teleconsultation with pharmacists or healthcare professionals.
  • Logging, encryption, and compliance (Law 25, HIPAA, GDPR, PCI-DSS).
  • Automated payment and billing module via Vaistat.
  • Full audit and traceability of operations and user access.

The key role of Health Proximity Agents (APS)

APSs represent the new generation of certified medical couriers. Trained according to strict standards (TMD, Law 90, Active Prevention Sheet, etc.), they become the human link between the hospital, the pharmacy, and the patient. During each accompaniment, the APS can also assess the patient’s behavior and well-being: were they calm? respectful? were they wearing their seatbelt? did they seem stressed or confused? These observations make it possible to enrich behavioral data and continually improve the patient and user experience.

An integrated, compliant and interoperable solution

Developed in Quebec and hosted on Azure Canada, the Vaistat platform complies with the highest security standards: Bill 25, HIPAA, GDPR, and PCI-DSS. It easily integrates with existing hospital and pharmacy systems (ERx APIs, EMR/EPR, RX systems). Each hospital, pharmacy, and healthcare provider or courier thus benefits from a unified view of the patient journey, facilitating interprofessional collaboration, error prevention, and real-time decision-making.
