The APS program

Health Outreach Agents

A new era for community healthcare
At Vaistat, we have a strong conviction: the healthcare of tomorrow must be more humane, more secure, and more accessible. It is with this vision in mind that we created the Healthcare Proximity Agents (APS) program, a pillar of Vaistat’s integrated care model.
The APS is not just a delivery person. They are trained and certified professionals at the heart of the care pathway, acting with responsibility, confidentiality, and proximity. Thanks to rigorous training and a comprehensive digital ecosystem, APS represent a new generation of professionals capable of meeting the challenges of pharmaceutical logistics, community monitoring, and post-hospital continuity.

Why the APS model is essential

The healthcare system is currently facing major challenges:

  • An aging population is increasing the need for home care.
  • An increase in prescriptions and medication follow-ups.
  • Pressure on hospitals, which must optimize their resources and reduce readmissions.
  • Risk of logistical disruption and loss of traceability between prescription and actual treatment administration.

In this context, the APS is becoming an essential link:

  • Ensures continuity of care upon hospital discharge.
  • Supports patients at home while ensuring secure medication delivery.
  • Builds trust between patients, pharmacists, and medical teams.
  • Ensures compliance with legal standards (Act 25, Act 90, HIPAA, GDPR, TMD)

The PAP program

Vaistat Post-Hospital Support
Through the PAP (Post-Hospital Support Program), Vaistat integrates the PAP into the critical phase of hospital discharge. When a patient leaves the hospital, the PAP acts as a pivotal partner between the clinical team, the pharmacy, and the patient’s home.

  • Transmission and validation of prescriptions between the hospital and the pharmacy.
  • Coordination of medication transport and delivery to the patient.
  • Personalized support upon discharge home (explanations, dosage reminders, and patient comfort).
  • Automatic notification to the pharmacist and clinician via the Vaistat platform.
  • Post-support follow-up: verification of treatment compliance and, if necessary, triggering a teleconsultation.

A unique and certified training course

Becoming an APS (Area of Professional Apprenticeship) means obtaining a recognized certification, structured around 13 comprehensive modules combining regulatory, technological, and human knowledge.

Technology and traceability

Each APS is supported by a smart digital ecosystem developed by Vaistat 2.0:
• APS mobile application: mission, real-time tracking, compliance, and proof of deposit.
• Blockchain traceability: each step is recorded and verifiable.
• Smart notifications: alerts synchronized between clinics, pharmacies, and patients.
• Legal compliance: all operations comply with applicable laws.

With the APS program, Vaistat is redefining the standards of local healthcare. The APS is not a delivery person; it’s a certified professional, guaranteeing confidentiality, traceability, and continuity of care. It represents the perfect fusion of human and technology, serving more accessible, more secure, and more human healthcare.
